How you learn is uniquely you

Everyone is unique, so everyone learns differently. This makes it hard for everyone to be happy in a class of 30 students everyday, all the time. During our tutoring sessions, I want to make the information relate to YOU. So, before we begin, I'll need to know a little more about you. 

General Background

This includes general information regarding likes/dislikes, etc. that'll I use in examples we discuss. 
Please print and complete before we meet, or request a copy during our first session.

Learning STyles

Besides what interests us, each person has their own preference of how they receive and process information. 
For example, are you an active learner to likes learning by doing, or are you more like me and like to reflect and digest information for it to make sense? Do you prefer any of these?
